Describe the environment and opening
Report whether the door or dock position is at ambient storage, a cooler, a freezer, or a transition between spaces. Include the installed equipment make and model if already available and the visible fault message. State the operational impact without assuming that every high-speed door or seal assembly suits the same environment.
Include the facility requirements
Explain service windows, access, sanitation or vendor onboarding requirements, and the facility contact responsible for approving the scope. Quote equipment and controls together
If an opening has an operator, activation devices, or dock interlocks, describe the connected systems. Ask the contractor which components its assessment covers, how parts lead times are determined, and how remaining limitations will be documented. For recurring work, keep service records at the individual opening level.
