Dock leveler not lifting: what to include in a service request

Document a stopped loading position without attempting to diagnose or repair energized equipment.

Describe the observable condition

A useful report distinguishes a deck that will not rise, a lip that will not extend, and a deck that moves but does not stay in its expected position. Write down exactly what the operator observed during normal use. Record a visible controller message from a safe location. Do not cycle the equipment repeatedly to reproduce a failure. The same observation can have several causes; the report is a starting point for a qualified assessment.

Identify the affected loading position

Give the facility address, building or suite, loading court, and bay number. If the nameplate is accessible without approaching damaged equipment, include the manufacturer, model, and capacity. Say whether the leveler is pit-mounted or edge-of-dock and whether the operating system is known. A photograph from a safe location can be useful, but the equipment description should stand on its own if images cannot be shared.

Explain the operational impact

Tell the provider whether a trailer is at the bay, whether the position has been removed from service, and whether freight can use another bay. State any visible leakage, impact damage, or related door and restraint faults. Your facility team decides how to control access under its procedures. A service request does not authorize continued use or establish that the deck can safely carry a load.

Prepare authorization and access

Identify the on-site contact and the person who can approve assessment charges and repairs. Include a purchase-order requirement, service window, gate access, and vendor onboarding needs. Ask the provider to confirm the assessment charge, arrival window, equipment scope, parts availability process, and who will document return to service. Do not assume a telephone description establishes the repair cost.
